From the looks of the angle of the throwout levers, it is waaay out of adjustment. That is probably what is letting the "fingered plate" come up out of the slots.
The only way to adjust/replace it would be to take it off.
 
I was hoping to avoid taking it off because it can be a difficult task. I have rebuilt the older style PTO's in the past and they seem easier to adjust. Since I have the spare parts on hand, I think will put the older style on. Thanks for all the responses. Good advice as usual from the members on this forum.

It is not too difficult to get it off.
Just make sure you get all six set screws out, and spray the holes good with PB Blaster.

Take the snap ring off, set the brake disc back in place in the notches where they are supposed to be and give it a try. No need to remove the PTO.

I can move the brake disc around by hand just like it is in the pictures, lining the notches up without removing the snap ring. If I remove the snap ring and line them back up, what keeps the pressure applied. It is possible the wave washer is bad, or a clip is missing?

I found out there was an issue a couple of days ago when I hooked up a mower deck. Even with the PTO just like it is in the pictures with the belt attached and tightened, it would not turn.

I may be missing the basic understanding of how this works? With the PTO depressed it keeps the PTO from turning/engaging. When the pressure is released, it allows the PTO to turn, engaging the belt. I do realize the fingers should be in the notches and it doesn't take a lot of movement to to engage. Could the problem be the friction disc is not adjusted correctly and allowing the brake disc to much movement.

That PTO looks misadjusted, like the spring tension isn't tight enough to draw the brake disk into the notches.

I suggest a complete readjustment of the PTO for starters......
 
I've been out on the 128 (Bull) and the engine wants to start acting up. I'll be cruising at about 2000 rpms in second with the engine running nicely then all of the sudden it sounds like a mini version of a jake brake. It sounds like the engine is in a pull. This only lasts for a second or two then it starts running fine again. I've scratched my head for a while and figure it might be in the valves but why does it come and go(?).

Anyone have this issue in the past?

Charlie, I have to disagree about not removing the pto. Like Steve pointed out, the springs aren't under much pressure at all. I bet once it's out the problem will jump out and be known. My guess is the nuts have loosened or fallen out. When properly adjusted the thrust button will be much further in and the braking plate should stay in the notches.

Michael taking off and rebuilding a mechanical pto is not a daunting task as it may seem just get quality rebuild parts from Charlie not the junk on greedbay I can speak from experience I had to rebuild my pto on my 125,wish I had bought O/E parts

The method to my madness on telling what I did was to remove the plate and more than likely everything else would fall apart in pieces. That would be a good way to see what everyone was talking about. Then he could look at the link which he obviously did not do after Mike F. gave it to him.
