We're waiting for a reply from David S., but to me your Trunion repair looks good. I am wondering if you replaced the 2 springs and buttons. The springs will loose tension over time and the buttons do wear which can cause erratic operation.

Yes all of that hardware is new and has been replaced. I appreciate your time on this
 
Looks OK.
But I would like to have seen the 4 inside corners a bit more square.

It’s very frustrating. I’m not sure what I’m doing wrong. Could the cam plate hole be worn out? There is no sliding where I put the shim washers but I can wiggle the plate from top a lot
