Leaking around the valve. Shut it off and leaks worse.
 
I got one from CC Specialties for my 782 plastic tank. My local Kohler guy had it too. As a short-term temporary measure, I used some PCV grommet. You need to determine if yours is leaking through the shut-off, or around it. Replace the bushing with the right item either way.

Yes, but you have to pull like really want it to come out. Maybe use a nail puller? My bushing broke on the way out, so that required tank removal, and considerable shaking.
 
That just doesn't seam well engineered, but I've never had one leak. I have a 2284 that will leak from the crossover if I fill it completely. I've wallered around on the floor looking for the solution and thus far the solution is 3/4 tank of fuel!
